Contact for Updates:
The information about the costs of the card described in this application is accurate as of October 1, 2020. This information may have changed after that date. To find out what may have changed, call us at 1-800-641-5046 or write us at Equitable Bank, Card Services, P.O. Box 160, Grand Island, NE 68802.
Billing Rights:
Information on your rights to dispute transactions and how to exercise those rights is provided in your Credit Card Agreement.
Important Information About Procedures For Opening A New Account:
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means to you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents. The information that is being requested and observed is for compliance with the requirements of Section 326 of the USA PATRIOT Act related to implementing customer identification and verification requirements.
Variable Rate Information:
Unless an Introductory APR is in effect, the Purchase APR and Balance Transfer APR is determined by adding 6.99%, 8.99%, 10.99%, or 12.99% to the US Prime Rate. The Cash Advance APR is determined by adding 14.99% to the US Prime Rate. The US Prime Rate is subject to change monthly on the first day of each month and is the US Prime Rate published in the “Money Rates” section of the Wall Street Journal on the last business day of the month. An increase or decrease in the US Prime Rate will cause a corresponding increase or decrease in the variable rate on the first day of your billing cycle that ends on or after the date of the change. Your actual APRs will be determined based upon your creditworthiness and will be provided to you in writing when your application is approved.
Rewards Program Information:
All accounts granted a credit card account will be enrolled in Equitable Bank’s Rewards Program. In this program, cash back will be earned based on use of the card. Cash back is awarded and redeemed as follows: In January of each year, your account will be credited an amount equal to 1% of your purchases for the previous 12 months using the credit card. See additional information about this program in your Card Agreement.
